What ApoB measures
Apolipoprotein B, or ApoB, is a protein that sits on the surface of the atherogenic lipoproteins — the cholesterol-carrying particles, chiefly LDL, that can lodge in artery walls. Because each of these particles carries exactly one ApoB molecule, measuring ApoB effectively counts the particles rather than just the cholesterol they carry. That distinction is why it is increasingly viewed as a sharper predictor of heart risk than LDL cholesterol alone.
Why it matters
It is the number of atherogenic particles, more than their total cholesterol content, that appears to drive the build-up of arterial plaque. The evidence supporting ApoB as a strong cardiovascular risk marker is good and growing, and many specialists now favour it when LDL and particle number diverge. A high ApoB signals a larger fleet of particles capable of entering the artery wall.
The levers that move it
Diet can lower ApoB, with several well-supported levers:
- Reducing saturated fat — and replacing it with unsaturated fats such as extra virgin olive oil — lowers atherogenic particles. The evidence is strong.
- Soluble and viscous fibre binds bile acids and helps clear cholesterol. Beta-glucan from oats, and the fibre in legumes like lentils and chickpeas, have good supporting data.
- Plant sterols, found in nuts, seeds and vegetable oils, modestly reduce cholesterol absorption.
The Mediterranean diet and DASH patterns bring these elements together and are well regarded for cardiovascular health. One honest caveat: where ApoB is meaningfully elevated, statins and related medication are the clinical mainstay, and they lower it far more powerfully than food can. Diet is a valuable adjunct, not a substitute for treatment when treatment is indicated.
The reference range
An ApoB below 80 mg/dL is often cited as optimal, with lower targets for those at higher cardiovascular risk. Treat this as a general reference rather than a personal goal — the right target for you depends on your overall risk and is best set with your doctor.
